THE STAFF
DR. KYLE SLEIGHT, DNP, FNP-C

Kyle is the founder, co-owner, and nurse practitioner at Maple Mountain Clinic. With more than 18 years of healthcare experience, he has dedicated his career to helping patients improve their health and quality of life. He earned his Doctor of Nursing Practice degree from Rocky Mountain University and spent many years working in the operating room before finding his passion in neurosurgery at Utah Valley Hospital.
Throughout his career, Kyle has been known for combining clinical expertise with a compassionate patient-centered approach. His interest in mental health health grew from personal experience supporting his wife through postpartum depression, which inspired a deeper commitment to helping individuals struggling with depression, anxiety, PTSD, and other mental health challenges.
As the medical provider behind Maple Mountain Clinic, Kyle is passionate about expanding access to innovative treatments that promote healing and hope. He strives to create a welcoming environment where patients feel heard, supported, and empowered throughout their healthcare journey. Outside of the clinic, Kyle enjoys spending time with his wife and two daughters, as wella s hiking, camping, hunting, golfing, and exploring Utah's beautiful outdoors.
ASHLEY SLEIGHT, RN, RDH

Ashley is the office manager, co-owner, and registered nurse at Maple Mountain Clinic. With roots spread across Utah, she proudly calls Delta, Utah, her hometown. After graduating in 2015 with a degree in dental hygiene, she met her husband, Kyle, and together they built a family while Ashley enjoyed a successful career in dental hygiene for eight years. When physical demands led her to seek a new path, she embraced her passion for healthcare and pursued nursing.
This past year, Ashley has worked as a registered nurse in mental health for the State of Utah, gaining valuable experience supporting individuals through some of life's most challenging circumstances. Her time in mental health deepened her understanding of compassionate, patient-centered care and strengthened her passion for helping others improve their well-being.
Recently, Ashley made the exciting decision to leave her state position and join Maple Mountain Clinic full-time, allowing her to focus on the patients and mission she cares about most. She helps guide patients through every step of their ketamine therapy journey, from scheduling and treatment preparation to payment coordination and ongoing support. Ashley also specializes in weight loss services, including GLP-1 medications and lipotropic injections.
Known for her warm personality and genuine care for others, Ashley strives to make every patient feel welcomed, supported, and understood. Her goal is to create a positive experience where patients feel confident in their treatment and empowered in their journey toward better health.
SAVANNAH COLLARD, BSN, RN

Savannah is an infusion nurse and the primary aesthetic injector at Maple Mountain Clinic. Since becoming a registered nurse in 2021, she has built a diverse healthcare background that includes medical-surgical nursing, IV therapy, and neurosurgery. Her healthcae journey began as a phlebotomist while attendding nursing school, where she developed a strong foundation in patient care and passion for helping others achieve their health and wellness goals.
Throughout her career, Savannah has been known for her attention to detail, clinical expertise, and ability to. help patients feel comfortable and confident in their care. As an experienced Botox injector, she combines artistic precision with a patient-centered approach to help patients achieve natural-looking, refreshed results. She also plays an important role in providing infusion therapies, ensuring patients receive safe, compassionate, and individualized care throughout their treatment experience.
Savannah is passionate about creating a positive and welcoming experience for every patient who walks through the door. She believes in treating the whole person, not just the condition being addressed, and strives to help patients feel supported through their wellness journy. OUtside of the clinic, Savannah enjoys spending time outdoors hunting, camping, paddleboarding, riding dirt bikes, horseback riding, reading, and pursuing creative projects with family and friends.
LOCATION

Since opening in November 2023, we have been honored to build strong connections within our community and witness the life-changing impact of ketamine therapy firsthand. What began as a vision to provide affordable access to ketamine treatment has grown into a thriving practice dedicated to helping individuals find hope, healing, and improved quality of life. As our patient community expanded, so did our commitment to meeting the growing need for compassionate, evidence-based mental health care.
In May 2026, we took an exciting step forward by relocating to Mapleton, allowing us to better serve our patients in a larger, more welcoming space. This move reflects the growth we have experienced and our continued dedication to making ketamine therapy accessible to those who need it most.
Looking ahead, we remain committed to expanding our services and reaching even more individuals seeking relief and healing. We plan to introduce innovative treatment options, provide comprehensive support, and foster a compassionate environment where every patient feels valued and understood. Our mission to improve lives remains at the heart of everything we do, and we look forward to continuing to make a meaningful difference in the health and well-being of our community.
